NEON is a successful recipient of a grant funded by the Health Resources and Services Administration, in which Lisa Ladendorff, NEON’s Executive Director, will evaluate and direct the community based project.

The Wallowa County Patient Activation Project is designed to address specific community health concerns (speculated with NEON’s local Community Health Assessment Data from 2011) by utilizing Continuous Quality Improvement disciplines and interventions by Behaviorists in a primary care setting to reach the following goals:

Ready or not, it’s that time of year again.  Out with the old, in with the new.  A new year, a new opportunity to get a fresh start.   For the last four millennia, we humans have been marking the New Year by vowing to get rid of some of our bad habits and generally improve our behavior.  Back in 2000, B.C. the Babylonians vowed to pay off all their debts and return all the stuff they borrowed during the previous year.  This New Year most of us Americans will vow to lose weight, adopt healthier habits, save more money and get more organized.  But, unlike previous years, this time we’re actually going to do it.  Really.  Seriously.  We mean it, this year will be different.

Quarterly Informational Call: National Rural ACO (Lynn Barr)

Please save Monday, December 16, 2013 @ 2:00 PM ET for the NCHN Informational Call.  We are pleased to announce that we will have Lynn Barr, Executive Director, National Rural Accountable Care Organization (NRACO) as the presenter.

As the US Health System transforms from fee-for-service to a fee-for-value model, rural providers have struggled to participate in innovative payment models due to low volumes, lack of infrastructure and programmatic barriers. The National Rural Accountable Care Organization was formed by rural hospitals, physicians and FQHCs to provide a cost-effective vehicle for interested providers to participate in the Medicare Shared Savings Program (MSSP), gain data, knowledge and experience in managing their chronically ill populations and get higher reimbursement for better outcomes, higher quality and patient satisfaction. Ideally suited for regional networks, the NRACO is recruiting participants for the July 2014 MSSP application. This session will describe the MSSP, the barriers and opportunities for rural providers, and the NRACO option for participation in the MSSP.

Rural Health Network Development Planning Grant Program (HRSA-14-043)
The purpose of ORHP’s Network Development Planning Program is to assist in the development of an integrated healthcare network. This program brings together key parts of a rural health care delivery system, particularly those entities that may not have collaborated in the past under a formal relationship. By working together, rural health providers and organizations can achieve efficiencies, increase capacity and improve the quality of essential health care services over a given geographic area.
